1. Provisional Allotment Letter.
   
2. NEET (UG)- 2023 Result card/ Score Card as applicable.
   
3. NEET (UG)- 2023 Admit card as applicable.
   
4. Matriculation certificate.
   
5. Passing certificate of 10+1 (Medical).
   
6. Detailed marks sheet of 10+2 (Medical).
   
7. Character certificate.
   
8. Residence/ Domicile Certificate (Only for State Quota Students).
   
9. Reserved category (SC/ ST/ OBC/ EWS/ Any Other) certificate as
   applicable.
   

10.Person with Disability (PwD) certificate as applicable as on

1. Migration Certificate (Original) (Digi Locker not acceptable*).
   
2. Proof of Identity (Adhaar/ PAN/ Driving License/ Passport).
   
3. GAP Certificate.
   
4. Exemption (if any).
   
5. Affidavit by the parents (if aspirants is minor) or by the Candidate himself/herself (if major) as published in the NEET UG-2023s Prospectus (Page No. 61).
   
6. 6 Passport Size Photographs.
   

One set Self attested photocopy of all above documents.
