1. Provisional Allotment Letter.
2. NEET (UG)-2022 Result card as applicable.
3. NEET (UG)-2022 Admit card as applicable.
4. Matriculation certificate
5. Passing certificate of 10+1 Physics, Chemistry, Biology and English.
6. Detailed marks sheet of 10+2 Physics, Chemistry, Biology and English.
7. Character certificate.
8. Reserved category (SC/ST/OBC/EWS/Any Other) certificate to be made as per format given in Prospectus annexures.
9. Person with Disability (PwD) certificate as applicable.
10. Migration Certificate (Original)
11. Proof of Identity (Aadhaar/PAN/Driving License/Passport).
12. Exemption (if any)
13. 4 Passport Size Photographs.
14. One set Photocopy of all above documents.

List of documents required for Verification purpose for State Quota Students

1. Provisional Allotment Letter.
2. NEET (UG)-2022 Result card as applicable.
3. NEET (UG)-2022 Admit card as applicable.
4. NOC issued by the Principal of the concerned college where he/she was studying MBBS/BDS last year(As applicable).
5. Matric (10th Class Certificate) for verification of age.
6. 10+1 Detailed Marks card for verification that candidate has studied Physics, Chemistry, Biology and English.
7. 10+2 Detailed Marks Card for verification of Eligibility as per Punjab Govt. Notification and NEET-UG-2022 Bulletin.
8. Punjab Residence Certificate
9. Reserved category (SC/ST/OBC/EWS/Any Other) certificate to be made as per format given in Prospectus annexures.
10. Migration Certificate.
11. Exemption certificate (if any)
12. Other Reserved category certificates.
13. Certificate from the Head of the institute where passed 10+1 and 10+2 examination.
14. GAP Certificate
15. Affidavit by the parents (if aspirants is minor) or by the Candidate himself/herself (if major) as published in the NEET UG-2022 Prospectus (Page No. 67).
16. Passport Size Photographs.
17. Proof of NTA(National Eligibility Cum Entrance Test UG -2022 Confirmation Page) form
18. One set Photocopy of all above documents.
